Mississippi Medicaid ASD rate update SPA 25-0003 began as a proposed amendment for rates effective December 1, 2025. Mississippi's late-breaking notice later says updated ASD rates will be paid for those service dates and earlier submitted claims will be reprocessed. The official state sources reviewed August 24, 2026 do not display a CMS approval for 25-0003, so the live operational notice, current schedule, and federal approval status must remain separately labeled.

Preserve proposal and operation as separate evidence
The December proposal notice describes what Mississippi submitted to CMS; it is not an approval letter. The later operational notice supports rate use and reprocessing, while the state SPA repository should be monitored for approval. Save each source, date, document state, and affected service period. Ask Mississippi for the approval package or written implementation authority when legal or audit review requires it.
Use the current official schedule
The fee-schedule page lists a current ASD schedule. Archive the file with retrieval time and connect each claim to code, unit, maximum, prior-authorization indicator, age range, and effective date. Keep previous schedules for services before December 1 and for any corrected claim. A spreadsheet display is not proof of member eligibility, medical necessity, authorization, claim acceptance, or payment.
Track reprocessing as a claim state
Build explicit states for originally paid, denied, suspended, identified for automatic reprocessing, adjusted, unchanged, provider action requested, appealed, and resolved. Do not submit duplicates merely because the notice says claims will be adjusted. Compare original claim control number, service date, code, units, allowed amount, remittance, adjustment reference, and net payment before deciding whether payer follow-up is needed.
Confirm program and payer route
Mississippi's ASD program page describes EPSDT eligibility, ASD diagnosis, medical necessity, and prior authorization. Confirm fee-for-service or MississippiCAN responsibility, plan enrollment, network status, authorization vendor, code, provider, and claim route. A state rate update does not automatically replace an MCO contract or authorize a service. Obtain written plan direction when the routes differ.
A fictional Mississippi adjustment audit
Avery locks 44 affected claims. Thirty-two have the operative notice, exact schedule row, prior-rate comparison, authorization, payer route, original remittance, adjustment state, and approval-status recheck. Completeness is 32 of 44, or 72.7%. Four were resubmitted unnecessarily, three lack the original remittance, two use the wrong schedule, one assumes CMS approval, one lacks MCO direction, and one changes units during comparison.
Escalate the source gap accurately
Describe the gap narrowly: the state has announced and operationalized updated rates, while the reviewed public repository does not yet show a final CMS approval package for SPA 25-0003. Do not call the rates invalid or the SPA approved without authority. Route the question to Mississippi Medicaid and counsel, preserve the response, and update the page when a final approval, withdrawal, or replacement appears.
