Archive the approved payment authority

Save the approval letter, Form 179, incorporated page, submission and approval dates, and earlier superseded page. The approved text says state-developed rates are the same for governmental and private providers and identifies the schedule route. It also contains historical rate-setting language. Do not infer the dollar change from an approval summary; compare dated schedule rows and retain the exact file used.

Use the current schedule by service date

Georgia's fee-schedule page lists the ASD schedule separately. Map code, modifier, unit, allowable amount, effective date, provider type, and any limit. Preserve the preceding schedule for earlier services and test corrected or late claims by the original date of service. A posted maximum does not guarantee the billed amount, allowed units, or payment.

Keep coverage and rate separate

The current ASD provider-manual repository controls program operations such as member criteria, recommendation, assessment, plan, provider qualification, prior authorization, documentation, and billing. A listed rate does not establish any of those facts. Qualified clinicians determine the individual recommendation; payer and operations teams verify authorization and code mapping without turning a price into coverage.

Identify fee-for-service or CMO responsibility

Confirm Georgia Medicaid or PeachCare eligibility, product, CMO enrollment, network provider, authorization owner, claim destination, and contract before using the schedule. The managed-care resource page identifies Georgia's CMO structure, but each plan's operative provider materials still require verification. A state fee schedule may not equal a contracted rate or establish out-of-network payment.
