Make a specific request

Keep the patient directly involved

Use speech, sign, writing, captions, screen-reader-ready text, interpreter, relay, assistive listening, or AAC. A family member can support when appropriate while the health care entity retains its communication duties and the patient's privacy and choices remain central.

Confirm the operational details

Record the request date, visit, communication task, requested aid or service, contact, response, arrival instructions, backup, and escalation route. Include the time by which confirmation is needed for the planned visit. Reconfirm when the visit type, platform, location, procedure, or communication need changes.

Sam requests access for six appointments. Five have a confirmed method, responsible contact, arrival note, and backup. One video visit lacks caption confirmation. Access readiness is 5 of 6. The appointment remains visible while the clinic resolves the request.
