Match the document to the visit

Check patient, clinic, clinician, visit date, document date, encounter type, page count, and portal or paper source. Keep a prior summary separate. A generated document can contain carried-forward information, so identify which sections changed at this visit.

Build a next-step list

Extract each medicine instruction, test, referral, appointment, home-care direction, warning sign, result contact, and due date without changing the wording. Preserve the section and page where each action appears. Assign the patient, caregiver, clinic, pharmacy, laboratory, insurer, or other responsible party only when the summary or verified follow-up supplies that role.

Ask about unclear or conflicting information

MedlinePlus recommends asking questions until information is understood, requesting written instructions, and learning how to contact the provider later. Mark discrepancies between spoken discussion, medication list, summary, and patient understanding for clinical clarification.

Keep access and record rights distinct

Provide large print, accessible electronic text, interpreter support, or AAC for review. HHS access guidance describes HIPAA access rights for PHI in a designated record set from covered entities. Access alone does not interpret the record.

Eli reviews ten listed actions. Seven have a responsible party, due date, and understood instruction. One medicine entry conflicts with the label, one referral lacks a destination, and one warning instruction is unclear. Action clarity is 7 of 10; all three return to the clinic.
