How can an ABA practice enroll with Virginia Medicaid and submit ABA authorization? Complete the MES provider application for the correct entity, individual, specialty, and location, then establish Cardinal Care participation or the applicable fee-for-service route. Use current assessment, setting, hour-justification, telemedicine, and authorization rules. Keep the July 2026 changes out of production until CMS approval and finalized manual updates take effect.

Start with the controlling delivery route

Virginia's provider enrollment portal supports new enrollment, status, revalidation, and provider training. It directs applicants to the pre-checklist by program, enrollment type, provider type, and specialty and assigns an application tracking number. If managed-care participation is selected, the application can flow to the chosen MCOs, while each final network state still needs confirmation.

The December 2025 ABA clarification remains the current operational source for in-person initial assessment, clinic-setting justification, support for requests over 20 hours, and telemedicine rationale. The July 28, 2026 policy notice says the new limit and diagnosis provisions await CMS approval and finalized manual changes; the authorization process remains unchanged until then. The provider library supplies the current manual route.

Keep enrollment and service gates separate

Maintain Virginia rows by group, individual, service location ID, MCO or fee-for-service route, service, setting, and modality. Track MES enrollment, provider specialty, affiliation, contract, credentialing, roster, eligibility, authorization, assessment mode, clinic rationale, hours review, telemedicine rationale, claim receiver, and revalidation. Proposed changes receive a future-state status.

Use verified, pending, held, and expired as the four Virginia workflow states. Each state should identify the decision owner, authoritative source, scope, effective period, last check, evidence, and next action. Automated checks can detect missing or conflicting values. Enrollment staff, plans, qualified clinicians, billing specialists, and legal or compliance owners make the decisions assigned to their roles.

Build the provider enrollment file

Use the MES pre-checklist and active-provider extract before opening a duplicate application. Preserve the application tracking number, ownership, tax, NPI, taxonomy, license, certification, locations, individual-group associations, screening, EFT, approval, and effective date. If joining Cardinal Care plans, store every contract, credentialing decision, roster, product, location, rate, directory result, and effective date separately. Test secure portal access and claim routing for each selected plan.

42 CFR 455.410 requires state Medicaid agencies to screen enrolled providers and enroll covered ordering or referring professionals. 42 CFR 438.602(b) assigns state enrollment, screening, and periodic revalidation duties for managed-care network providers. Its one limited pending-network-agreement period can last up to 120 days, though it supplies no billing effective date or payment promise for a Virginia provider. The CMS NPI fact sheet explains that an NPI identifies an individual or organization; licensing, credentialing, enrollment, contracting, roster status, authorization, and payment each need separate proof.

Make the configuration record usable

Give each Virginia row a durable identifier. Use one row for every relevant combination of billing entity, rendering role, location, payer or program, product, service, setting, and submission route. Fields should cover legal name, NPI, tax identifier where needed, taxonomy, license or certification, state provider number, screening, revalidation, contract, credentialing, roster, directory, portal role, authorization receiver, claim receiver, effective dates, source version, and responsible owner. Attach the document or transaction supporting every release-critical field.

Build three operational views from the same Virginia record. The launch view shows incomplete provider, plan, access, and claim-test work. The client release view joins member eligibility, delivery route, provider and location, qualified clinical decision, authorization, schedule, and units. The reconciliation view links the original claim, acknowledgments, adjudication, remittance, deposit, correction, refund, and recoupment. Restrict sensitive data by role, preserve change history, and hold only the affected configuration when evidence conflicts.

Configure authorization for the member

Verify the member's current Cardinal Care plan or fee-for-service route. Build the request from an in-person initial assessment when current policy requires it, individualized plan, provider, service, setting, dates and units, and supervision. Add specific clinic justification when clinic care is requested, detailed medical-necessity support above 20 hours, and telemedicine justification when relevant. Preserve receipt, questions, decision, and renewal. Leave pending 2026 criteria outside the live checklist.

Release claims from the service record

Virginia claim release should compare MES and MCO status, member route, authorization, provider and service location, assessment and setting evidence, actual time, code and modifier, units, supervision, modality, and note. Configure Acentra or each MCO receiver according to the current route. Reconcile rejection, adjudication, remittance, and deposit, then link corrections to the original claim. A future-rule test belongs in a sandbox configuration.

A fictional launch review

A fictional Richmond practice reviews 18 MES-plan-setting rows. Twelve are ready. Two MCO rosters remain pending, one individual-group association is missing, one clinic request lacks setting rationale, one live rule applies the unapproved July change, and one claim receiver is untested. Readiness is 12 of 18, or 66.7%.

The Virginia example fixes its denominator before review begins. A submitted application, user account, directory listing, unrelated approval, or successful claim at another site leaves the held row in the denominator. The owner records the exception, responsible person, due date, next action, and evidence required for release.

Monitor the live workflow

Review the provider portal, ABA clarification, provider library, MCO sources, and July-change status monthly. Measure MES decisions over applications due, plan rosters effective over plan rows due, requests with complete setting and hour rationale over applicable requests, authorization decisions by target over requests due, and mature first claims adjudicated without resubmission over mature first claims.

Keep a Virginia future-policy readiness worksheet outside the production rule table. It can list likely affected authorizations, provider education, portal fields, data needed for hour aggregation, and unresolved questions. Each item should carry a source and dependency. Move a field into production only after the state publishes the required approval, final manual language, and effective date, then validate it against a fictional Cardinal Care and fee-for-service case.

Keep a dated Virginia change register. For each new notice, manual, fee file, form, contract, or portal instruction, record whether it is current, future, proposed, superseded, or archived; identify affected configurations; test the change on approved fictional data; and document who approved production use. This keeps source maintenance observable instead of relying on staff memory.

Go/no-go review before covered service

  • MES provider type, specialty, location, and affiliations match the service.
  • Cardinal Care or fee-for-service routing is confirmed.
  • Current assessment, setting, hours, and telemedicine rules are satisfied.
  • Pending July 2026 changes remain outside production.
  • The authorization and claim use the same provider, setting, dates, and units.

A go result applies only to the named Virginia configuration and service period. When a license, enrollment, contract, roster, authorization, source, or claim control expires, pause new covered-service commitments for that row. Route current clients through qualified clinical, payer, access, and continuity review under the applicable rules.
