Prove the Oscar product before choosing the Optum lane
Oscar publicly offers individual health plans in New York, and its 2026 provider manual identifies the New York design as an EPO. That makes the current card and product evidence operationally important: an Oscar logo does not by itself prove the New York individual EPO, a particular network, an ABA benefit or permission to use an out-of-network provider.
At intake, save readable images of the card's front and back together with the member and group identifiers, exact plan and network labels, coverage dates and plan document. The same file should identify the provider entity, TIN, billing and rendering NPIs, clinician, service location, proposed service and dates. Product identification should also answer who administers each part of the case. Oscar's manual says Optum is nationally delegated for behavioral-health and substance-use services and describes Optum review, appeal and claim responsibilities. Oscar retains other medical and plan-administration functions. The distinction is not cosmetic: a staff member can receive a correct answer from Oscar about medical benefits and still need Optum's current ABA requirement, network and claim instructions.
This page is narrower than the general Optum commercial guide and different from the Oxford and Healthfirst Leaf New York guides. Their portals, agreements, payer identifiers and notices cannot fill a gap in an Oscar record. When the card, product page and authenticated response conflict, the safest next step is written clarification from the responsible administrator, not an assumption based on another New York commercial plan.
Treat Oscar and Optum participation as separate evidence
Oscar's 2026 welcome kit tells providers that behavioral-health and substance-use coverage is administered through Optum and that participation with the specified vendor is necessary to serve Oscar members as in network. Those sources support a two-sided onboarding file. On the Oscar side, preserve the legal entity, organizational NPI, TIN, locations, Oscar credentialing or contracting records, roster status and effective dates. On the Optum side, preserve the behavioral-health application, credentials, agreement, product or network loading, clinicians, sites, portal access and written effective participation. Oscar medical participation, a CAQH profile, an Optum login or a directory appearance is not a substitute for the complete set.

A concise internal status note can prevent scheduling mistakes. It might say that the practice's Oscar medical agreement is executed, while Optum ABA participation for the new site remains unconfirmed. That language is more accurate than “in network” without a product, entity, site and effective date. Any change to ownership, TIN, NPI, clinician, credential, address, modality or network should reopen only the affected participation questions and preserve the earlier evidence.
Use the specific Optum request route for ABA
Oscar's general materials increasingly use Availity for medical authorization. The 2026 welcome kit says all authorizations move to Availity on September 1, and Oscar's public prior-authorization page points providers to current tools and lists. Its specialty-routing page explicitly sends behavioral-health and specialized-therapy requests to Optum Provider Express. That specific delegated instruction is the operational rule for this ABA guide.
Optum's ABA hub describes secure Provider Express and One Healthcare ID workflows for verifying ABA eligibility and coverage, requesting assessment or treatment authorization, sending additional information and checking status. Optum ABA provider resources Optum's prior-authorization page says ABA follows a two-step assessment-then-treatment process for most plans and tells providers to verify benefits before service. Oscar's Individual Prior Authorization List effective July 1, 2026 places 97151 and 97152 in the adaptive-behavior assessment group and 97153 through 97156 in treatment. It does not establish a universal answer for later dates, an unlisted code, a modifier, a different setting, a particular clinician or a nonindividual Oscar product.
One timeline can make the request legible without turning it into a checklist. It should show when staff checked the requirement, when the clinician approved the assessment packet, how it was sent, when Optum acknowledged it, what information Optum later requested, how the practice responded and what the written assessment or treatment decision actually covered. Keep clinical judgment with the qualified clinician
The Optum supplemental criteria begin with a crucial limitation: the member's benefit document and applicable law supersede the general criteria. The document is informational and does not replace professional judgment. Within that boundary, the current criteria describe categories a clinical request may need to address, including diagnosis support, assessment, functional information, measurable goals, treatment planning, caregiver or other coordination, progress and transition. The qualified clinician decides what is clinically accurate and appropriate for the person. An operations team may identify an empty field, connect the right records and securely transmit the approved packet; it should not write a diagnosis, select an intensity, manufacture progress or change a goal to resemble payer wording.
Oscar publishes a clinical-guidelines page, but it also identifies Optum as the behavioral-health manager. If an Oscar page, Optum document and written case instruction appear inconsistent, preserve all three and ask the responsible administrator for a written resolution.
New York professional authority remains an independent gate. The State Education Department provides a public tool for verifying a licensed behavior analyst or certified behavior analyst assistant and current registration. Translate the written decision into a service control
A favorable Optum decision deserves a line-by-line comparison with the submitted request before recurring appointments are opened. The practice should retain the member and product, request or authorization number, assessment or treatment phase, service and code scope, provider and site if specified, effective dates, units or other quantity, frequency, conditions, continuation requirements and every unresolved item. Any difference between the request and decision belongs in a dated clarification queue.
The rendered record then needs to reflect what actually occurred. Optum's 2026 ABA documentation protocols describe record elements by service category, including member and service identifiers, provider credentials, place and date of service, actual time evidence when applicable, participants, intervention or assessment detail, data or narrative, signatures and supported corrections. Operations staff can compare scheduled visits with the written span and track remaining authorized quantity without making clinical decisions. The clinician remains responsible for accurate assessment, treatment, supervision, progress, caregiver involvement, coordination and transition reasoning. Dated amendments should leave the original record understandable rather than silently replacing it.

Send the delegated claim to the verified receiver
Oscar's provider manual and welcome kit identify Optum payer ID 87726 for delegated behavioral-health claims and list the current mailing address as P.O. Box 30757, Salt Lake City, Utah 84130-0757. Oscar's welcome kit separately identifies the general Oscar medical claim route and payer ID OSCAR. The two entries should remain distinct. A staff member should not send an ABA claim to the Oscar medical receiver merely because that route appears elsewhere in the same kit.
Before release, compare the final rendered record with the member, Optum receiver, billing entity, TIN, billing and rendering NPIs, location, authorization, dates, diagnosis linkage, service code, modifier, units and charges. Preserve the exact outbound version, clearinghouse response, Optum receipt, status, adjudication, remittance, adjustment or recoupment and bank deposit. Each stage answers a different question; a clearinghouse acceptance does not establish adjudication or payment.
The Oscar manual says delegated-vendor claims follow the vendor's guidance and makes timely filing subject to state law and the governing agreement. That is why this page does not publish one universal deadline. The case file should retain the current Optum agreement, claim instruction, receipt date, remittance or denial notice and deadline calculation. A failed claim should be classified before it is touched. Demographic or registration error, network participation, member benefit, authorization mismatch, code or unit support, clearinghouse rejection, payer denial, underpayment, coordination of benefits and recoupment have different evidence and owners. Resubmitting an unchanged claim can create a duplicate while leaving the original defect untouched.
Route reviews by the decision owner and notice
The Oscar manual assigns Optum responsibility for behavioral-health authorization reconsiderations, first-level member and provider appeals and certain second-level appeals. That makes the Optum notice, not the Oscar brand alone, the starting point for an ABA clinical or delegated-claim dispute. Preserve the decision owner, denial basis, notice date and receipt date, requested remedy, supporting record, standing and current submission instructions before choosing a route.
Oscar can still own a member or plan-administration question outside the delegated decision. The working file should separate an Oscar eligibility or plan-document issue, an Optum participation issue, an ABA authorization or clinical adverse determination, a rejected claim, a corrected claim, a payment dispute and a member appeal. If the notice is unclear, ask which entity issued it and request a complete copy rather than sending the same packet to multiple queues.
New York DFS explains that external appeal may be available for certain medical-necessity, experimental or investigational, and out-of-network denials. It generally gives a member four months after the final adverse determination or internal-appeal waiver and a provider appealing on its own behalf 60 days. In week two, the practice reconciles Oscar medical and Optum behavioral-health participation for the entity, clinicians and sites. In week three, the clinician approves an assessment packet and staff submit it through the current Optum route. In week four, the team compares the written response with the request, documents any gap and opens only supported services. No step is treated as proof of the next.
