Pica

What should families know about Pica and safety? Pica is a pattern of eating nonfood material. Any suspected ingestion can require immediate poison, choking, or medical action, even before a diagnosis applies. Families should secure hazards, preserve communication and supervision, record the item, amount, time, and symptoms when safe, and contact the appropriate medical resource. Behavioral assessment begins after acute risk is addressed and cannot replace medical evaluation.

Pica and a single ingestion event are different questions

The MedlinePlus medical encyclopedia describes pica as a pattern of eating nonfood materials and says the pattern must last at least one month to fit the diagnosis. A qualified clinician also considers developmental level, cultural practice, pregnancy, medical conditions, and the material involved. Mouthing an object, chewing a safe item, and swallowing a nonfood substance should be recorded separately.

Safety action never waits for the one-month threshold. One swallowed battery, magnet, sharp object, toxic substance, contaminated material, or choking event can be an emergency. Families should report the exact observed or suspected item, amount, time, route, symptoms, age and weight when requested, and any packaging or photo that can be obtained safely. A label such as “sensory seeking” cannot determine toxicity or medical risk.

The U.S. Poison Help site connects callers to poison centers at 1-800-222-1222. The National Capital Poison Center advises calling 911 when a person collapses, has a seizure, has trouble breathing, or cannot be awakened, and advises contacting Poison Control immediately for a possible poisoning. Other countries use their local poison and emergency services.

Medical evaluation comes first

Pica can be associated with poisoning, choking, intestinal blockage or injury, infection, dental harm, and nutritional problems. Risk depends on the substance, amount, timing, contamination, shape, size, and the person's health. Absence of visible symptoms does not make every ingestion safe.

MedlinePlus says a medical professional may assess iron and zinc, anemia, lead exposure, or infection based on the history. A deficiency may coexist with pica, and a test result alone does not establish why an event occurred. The medical plan should state which signs require emergency care, poison consultation, same-day evaluation, monitoring, imaging or laboratory work, and follow-up. ABA staff should follow that plan within training and scope.

A 2021 study by Fields and colleagues analyzed children ages 30 to 68 months in the multisite Study to Explore Early Development: 1,426 with autism, 1,735 with other developmental disabilities, and 1,578 population controls. Study-defined pica was reported for 23.2% of the autism group and 3.5% of controls. These figures describe that preschool sample and measurement method; they are not a universal rate for autistic people of all ages.

Build an environmental and supervision system

Map risks by setting instead of relying on vigilance alone. A plan can include:

  • a pre-use scan for the specific materials and contamination risks in that location
  • secure storage, disposal, cleaning, and maintenance assigned to named adults
  • supervision and handoff rules that state who is responsible and when
  • safe food, hydration, oral-motor, sensory, or communication supports selected by the relevant professionals and person
  • an accessible way to request food, a chew item, help, a break, or removal of a hazard
  • an acute-response card with poison, emergency, medical, and family contacts
  • incident documentation, medical follow-up, and review of every environmental failure

Controls must preserve nutrition, hydration, bathroom access, mobility, AAC, dignity, and community participation. A locked cabinet may reduce access to one hazard while loose debris, peeling paint, soil, craft supplies, or staff transitions remain unaddressed. Check the full pathway.

Never place dangerous materials within reach to test treatment. Avoid punishment, shame, unpleasant substances, or response blocking that exceeds the current authorized, trained, least-restrictive safety plan. Any physical intervention needs its own clinical, medical, legal, consent, setting, and competency review.

Assessment cannot infer function from the item

After acute and medical needs are addressed, an appropriately qualified behavior analyst may define and assess observable patterns. Record the material, contact with mouth, ingestion when known, setting, opportunity, health and access conditions, communication, adult response, and consequence. Keep observed, reported, and unknown events separate. Soil ingestion does not automatically prove a sensory function, and removal of a demand does not prove escape caused the event.

Assessment may lead to environmental change, communication teaching, competing access, schedule change, caregiver training, or another individualized intervention. Medical, nutrition, dental, feeding, occupational, speech-language, and mental health professionals may have separate roles. The team should share defined evidence while retaining each profession's decision authority.

The ASHA AAC portal says AAC users should always have access to their communication tools or devices. Do not remove AAC to create motivation or require speech, eye contact, calm behavior, or task completion before honoring a recognizable request.

For BCBA and BCaBA certificants and people who have completed an application for either credential, the current BACB Ethics Code addresses competence, collaboration, consent and assent when applicable, medical variables, assessment, risk, restrictive procedures, documentation, and evaluation. Pica diagnosis and medical management remain outside an ABA clinician's scope unless the person separately holds the relevant license and authority.

Measure hazards, access, and response

Useful measures include completed environmental checks per required setting, assigned supervision per transition, AAC access, hazards found and removed, observed contacts and ingestions by material, response-plan steps completed, time to poison or medical contact, injuries and complications, and the person's report. No ingestion should be arranged as a probe.

Jamie is a fictional eight-year-old who uses AAC. Before ten garden sessions, the plan requires a site scan, named supervisor, AAC, and the medical response card. All four are ready in 8 of 10. One session has loose fertilizer accessible; another lacks the assigned handoff, so both are paused before entry.

During the eight ready sessions, adults observe three movements of nonfood material toward the mouth. They complete the authorized immediate safety response in 3 of 3. In one event, contact with the mouth is suspected, so the adult follows the medical card and contacts the designated medical or poison resource. These small descriptive counts cannot establish behavioral function or treatment effect. The team corrects the environment and routes assessment to the qualified roles.
