The notice separates three ideas
45 CFR 164.520 requires descriptions of specified authorization-dependent activities, the rule for other undescribed uses, and revocation. Use wording that fits the entity's practices and current law. Avoid implying authorization is required for every treatment, payment, or operations disclosure.
First identify any activity that needs a specific description under the current rule and whether the entity actually performs it. Then state that other uses and disclosures outside those described in the notice will occur only with written authorization, subject to current law. The revocation language completes the sequence by telling the individual that authorization can be withdrawn through the defined process.
Connect notice language to an activity inventory
Maintain an inventory of authorization forms and the workflows that rely on them. Include the purpose, PHI scope, recipient, expiration, required elements, system location, owner, and governing authority. Compare the inventory with the notice so staff do not use a broad notice sentence as a substitute for a valid authorization. Flag marketing, sale-of-PHI, psychotherapy-note, research, media, fundraising, and other specialized pathways for qualified review rather than assigning a category from memory.
Authorization may be only one possible route. A legally permitted use, required disclosure, treatment or payment pathway, business associate arrangement, or more protective rule can lead to a different analysis. The notice should orient the reader without promising that a signed form automatically makes every request permissible.
Revocation has a defined rule context
Explain how the individual can revoke and route the request to the responsible office. The notice should not promise retroactive effect or ignore actions already taken in reliance on a valid authorization. More protective law may add requirements.
Use a revocation intake that records the individual or representative, affected authorization, receipt time, permitted submission method, verification, downstream owners, and completion. Staff should be able to locate the authorization without asking the person to restate sensitive details unnecessarily. Route the revocation to systems, clinicians, vendors, researchers, billing teams, or other recipients whose future activity depends on it.
Define the effective operational event and preserve actions taken before that event under the applicable rule. A revocation request can be valid even when staff cannot stop every downstream action immediately. Escalate uncertainty, document the supported scope, and avoid telling the person that revocation erased an earlier lawful disclosure. Provide a clear response and an accessible contact for questions.
Keep related forms and decisions separate
Notice acknowledgment confirms receipt of the NPP. Treatment consent addresses care. Payer authorization concerns benefit administration. A HIPAA authorization permits specified uses or disclosures under its own requirements. Label the records distinctly, train staff on the decision path, and prevent a missing NPP acknowledgment from being reported as an authorization refusal.
Example with notice versions
Five active notices are tested. Four contain all three concepts; one describes authorization but omits revocation. Statement completeness is 4 of 5 notices.
The owner adds reviewed revocation language, tests the published phone and written routes, and checks whether the correction reached paper, web, portal, translated, and joint-notice formats. It also samples open authorizations to confirm that a real revocation can reach the systems using them. The notice defect closes only after content and operational routing both work.

Use HHS model notices as current drafting support, then tailor them. Maintain a legal source map, activity inventory, revocation instructions, contact details, effective date, and version testing across paper and electronic copies.
Monitor active authorization workflows, revocations received, routing age, unresolved scope questions, current notice coverage, and obsolete instructions. Audit from revocation records into downstream completion and from continuing authorization-dependent activity back to an active authorization. Preserve the authorization, revocation, decisions, notifications, and version history under appropriate retention and access controls.
Test the public instructions with a realistic revocation request. Confirm that staff can identify the affected authorization, preserve the original receipt, notify each future-use owner, and explain the supported outcome without asking the individual to repeat the request across departments.
