Families should verify the plan-specific request route, usable provider capacity, communication access, complete notice, appeal deadline, and any earlier continuation deadline before relying on an assessment, treatment, or renewal date.
Confirm the STAR product and service area
Texas HHS's managed-care service-area map assigns STAR plans by area. The map remains useful, but it predates the August 2026 FirstCare and Baylor Scott & White exit notice. Record Sela's current plan, Medicaid ID, STAR product, service area, address, effective dates, and service date from current eligibility evidence. A plan name or an older map alone cannot establish the active route.
Compare the state eligibility response, Parkland member account, and provider eligibility check for the exact proposed date. Save the date, channel, and reference number. If an old plan, address, or service area appears anywhere, resolve the discrepancy before sending records. That gives Sela's family a practical choice among proceeding through Parkland, correcting eligibility data, or planning for a documented transition. It also prevents a provider from treating a directory result as proof of current product enrollment.
Use Texas ABA policy with the named plan route
TMHP's managed-care autism update says MCOs must provide medically necessary Medicaid-covered services while administrative authorization, referral, and claim procedures can differ. The Children's Services Handbook supplies the state Autism Services framework, and the Managed Care Handbook describes STAR operations. Apply those sources with Parkland's current instructions for Sela's exact request.
Follow the current plan-specific authorization path
Parkland's current STAR page identifies the member product and explains that certain services require prior authorization. The current ABA policy summary requires authorization for ABA evaluation, initial treatment, and later reevaluation, while the code lookup asks for date, plan, and procedure code. Save the returned result because a remembered list may be stale.
Build one auditable request record
Sela's record should state the STAR product, service area, service date, assessment or treatment phase, requested codes, units, dates, settings, qualified provider, billing and rendering identities, supervising clinician, and every attachment. Add the submission receiver, receipt, missing-information request, written result, effective period, renewal trigger, and deadline. Keep relationship, consent authority, communication permission, and disclosure authority in separate fields.
Mark who created each document, when, and for what purpose. Clinical findings remain attributable to the clinician, family priorities to Sela and her caregivers, transmission evidence to the practice, and administrative results to Parkland. Use the secure submission path identified for the request and send only relevant records. Aquarium staff, school personnel, care managers, relatives, providers, and payer staff may each have a role, but that role alone does not authorize access to a full education, health, or family file. Record the specific permission used for every disclosure.
Keep decision authority clear
A qualified clinician evaluates Sela and authors recommendations within professional scope. Parkland issues its coverage or utilization decision. The legally authorized person gives consent when required, and Sela's assent applies when applicable. Operations may verify evidence and schedule a cleared event. A plan result does not create clinical authorship, consent, provider capacity, claim acceptance, adjudication, or payment.
Verify the full provider configuration
Ask the practice to verify current Texas Medicaid enrollment, Parkland STAR participation, billing and rendering identities, service location, provider type, supervision, requested codes, and effective dates. Then confirm actual staff and appointment capacity for home and a sensory-friendly aquarium program. A directory entry, credentialing file, contract, authorization, and available start date answer different questions. Save the source and date for each answer.
Separate the lookup, evaluation, and treatment states
Parkland's lookup uses service date, plan, and procedure code. Save that result as a dated rule check. Create different rows for ABA evaluation, initial treatment, reevaluation, and any continuation request. Each row needs its own provider, settings, codes, dates, units, receiver, receipt, information request, written result, effective period, and renewal trigger. A lookup result or transmitted form is not an approval. Use readable states such as preparing, submitted, received, incomplete, under review, partly approved, approved, denied, expired, or replaced so the family can see who owns the next action.
Release the exact event supported by evidence
Before Sela's assessment or treatment visit, recheck eligibility, STAR assignment, service area, provider and location status, authorization or other applicable result, staff, supervision, date, code, units, and setting. Confirm essential health and safety information, picture-based AAC, gesture, vocal approximations, and an all-done message, and an accessible way to accept, pause, or withdraw when applicable. Record the event released; another provider, code, or date needs its own support.
Ask whether the assigned team has a dependable home opening and permission to work at the aquarium program. The aquarium controls admission, crowd and animal-area rules, and site safety. The clinician controls clinical methods and supervision. Sela's family decides whether the proposed schedule and supports fit her needs and priorities. If Parkland's result covers an evaluation at home, it cannot be stretched to treatment or a community site without matching evidence. Keep each setting visible until its own operational gates are clear.
Resolve the realistic complication
If Sela's provider is outside Parkland's participating network, run the code check and the out-of-network request as separate gates. Ask whether the plan is reviewing clinical necessity, network availability, or both. A clinical approval cannot silently create a contract, roster date, available appointment, or payable claim path.
Escalate a network gap with a dated search
42 CFR 438.206 requires a Medicaid managed-care entity to arrange timely out-of-network coverage when its network cannot provide a necessary covered service, with enrollee cost no greater than in network. Sela uses picture-based AAC, gesture, vocal approximations, and an all-done message. Partners should know how she signals completion, allow time for a response, and honor a choice to stop or change activities. Assent remains meaningful throughout a visit, even when an authorized adult supplies formal consent. A lost child, animal-contact event, injury, sudden illness, or serious distress calls for the aquarium and family safety plans and appropriate medical help. Coverage approval does not make the payer the site-safety or clinical authority.
Read the notice and calendar the earliest deadline
For a Parkland adverse benefit determination, 42 CFR 438.402 generally gives a Medicaid managed-care enrollee 60 calendar days from the notice to request a plan appeal. The complete notice controls the affected service, dates, record route, expedited option, continuation conditions, and State Fair Hearing sequence. Earlier action may be required to preserve continuing services. Save Sela's full notice and its envelope or portal timestamp.
Use the route that matches the problem. A member appeal contests Parkland's adverse benefit determination. A complaint about access, communication, privacy, or service may belong in the grievance process. A provider contract, claim, coding, or payment dispute is separate and does not keep Sela's member appeal timely. Ask Parkland for an accessible notice and filing assistance if needed. When the treating clinician believes ordinary timing could seriously threaten health or functioning, ask what supports expedited review. The actual notice controls any earlier continuation deadline and the sequence for a Texas fair hearing.
Ask questions that produce a usable answer
Call the number on Sela's current Parkland card. Ask which STAR assignment and service area are active for the date, which ABA assessment or treatment rule applies, who receives the request, and what proves receipt. Ask which provider, location, staff, codes, units, and dates the written result covers; what remains open; which provider has confirmed capacity; and which appeal or continuation deadline is earliest. Request a reference number and written confirmation.
Measure a locked release workflow
Sela's team predeclares 22 checkpoints for home and a sensory-friendly aquarium program. 15 are complete and 7 remain visible holds, so readiness is 15 of 22, or 68.2%. The denominator includes every checkpoint due for this proposed release. This fictional measure establishes no eligibility, clinical appropriateness, coverage, network adequacy, appeal result, claim outcome, or payment for another member.
If Parkland confirms receipt of the evaluation request, one checkpoint closes and the record becomes 16 of 22, or 72.7%. The denominator stays 22. If the later letter approves the named evaluator, home setting, dates, codes, and units, a decision checkpoint closes and readiness becomes 17 of 22, or 77.3%. Initial treatment, the aquarium setting, provider staffing, and final communication checks remain open. The numbers make the limited business meaning of each document visible.

Before Sela starts the exact service, verify:
- active STAR enrollment, Parkland assignment, service area, and service date;
- the dated plan-and-code lookup plus the separate evaluation or treatment request state;
