Organize without changing the source
Store each file with holder, record type, covered dates, request date, receipt date, page count, and version. Preserve originals and add a separate index or plain-language question list. Use AAC and accessible formats for the person's review.

Tessa requests four record sets. Three arrive in the requested readable format with matching dates and page counts. One hospital request remains open. Receipt completeness is 3 of 4. The open request keeps its original submission and follow-up dates.
