North Carolina school Medicaid behavioral health 2026 expanded through approved SPA 26-0002, effective July 1, 2026. The public notice adds provider types and services including research-based behavioral health treatment, expands qualifying documentation beyond an IEP, IHP, BIP, or section 504 plan to another written treatment plan, and updates LEA cost and time-study methods. It does not make educational, duplicative, or medically unnecessary activity payable.

Identify the controlling written plan

Lock the student, Medicaid eligibility, medical need, IEP, IHP, BIP, section 504 plan, or other written treatment plan, author, effective dates, service, frequency, setting, and responsible clinical provider. An educational goal or school schedule alone does not establish Medicaid coverage. Preserve the exact plan section that supports the health service.

Verify the provider and service pair

Use the approved state-plan pages and current clinical coverage policies to match service, practitioner type, qualification, license, supervision, LEA employment or contract, enrollment, and date. Adding a provider category does not authorize every service within that person's general scope, and adding a service does not waive the provider-specific requirements.

Separate school Medicaid from RB-BHT treatment

A student may receive school-based behavioral health and separately authorized RB-BHT or ABA. Map goal, intervention, provider, time, setting, plan source, payer route, and outcome. Do not bill the same activity twice, supplant IDEA obligations, or convert teaching, tutoring, custodial supervision, transportation, staff training, or general classroom support into a medical service.

Build the LEA cost trail

The amendment uses interim payment, cost reporting, time study, Medicaid ratio, reconciliation, and an allowed administrative-fee mechanism. Preserve payroll or contract cost, participant eligibility, service evidence, time-study universe, exclusions, allocation, interim payment, settlement, and audit file. Retain the CMS school-based services guidance with that evidence. The fee schedules may govern specified services, but a listed amount does not replace the approved cost method.

A fictional North Carolina audit

Andre locks 42 school services. Thirty have eligible student, medical need, controlling plan, qualified provider, service record, time-study treatment, payment route, nonduplication check, and settlement link. Completeness is 30 of 42, or 71.4%. Three rely only on an academic goal, three duplicate outside treatment, two use an ineligible provider, one lacks medical necessity, one misallocates time, one omits the Medicaid ratio, and one has no settlement trace.

Protect student and family choice

Coordinate with the student and family using accessible communication. School delivery should not block access to another medically necessary provider or force disclosure beyond lawful need. If services overlap, the clinical and school teams should clarify responsibilities without delaying care. Preserve consent, privacy, notice, appeal, continuity, and the family's choice where applicable.
