Montana Medicaid CBHT 15-Minute Units: May 2026

Lock the approved effective date

Save the CMS letter, Form 179, approved pages, and state implementation notice used. The package repeatedly states May 9, 2026 even though a public result listing may display July 1. Treat that mismatch as a source-order issue and request Montana clarification if a portal, fee schedule, or claim edit uses another date. Do not backdate or rebill solely from an unsourced summary.

Identify the exact CBHT service

The amendment concerns the EPSDT Comprehensive Behavioral Health Treatment bundle and its payment unit. It does not make every Montana behavioral-health, ABA, psychotherapy, home-support, or youth service a 15-minute CBHT claim. Record member eligibility, program, provider type, licensed entity, service definition, treatment plan, code, modifier, place, date, and authorization. Use current youth mental-health materials to confirm the operational service.

Convert time without manufacturing units

Design a mapping from documented start and stop time to the current state unit rule, including rounding, overlapping staff, group delivery, travel, noncovered activity, interrupted sessions, midnight, and corrected claims. The SPA names the unit but does not answer every billing edit. Preserve delivered activity and time exactly. Obtain written state direction before converting a per-diem authorization or historical claim to units.

Reconcile SPA, schedule, and adjudication

Montana says adopted manuals and schedules appear under the corresponding provider type after the proposal and adoption process. Compare the approved SPA, provider resource page, adopted fee schedule, authorization, portal configuration, remittance, and later notice. A rate display is not coverage, and an approved service is not guaranteed payment. Record the source and effective date for each operational field.

A fictional Montana conversion audit

Sage locks 30 CBHT workflows spanning authorization, documentation, and claims. Twenty-two have the approved effective date, exact service identity, current unit rule, documented time, conversion logic, rate source, test claim, and exception owner. Completeness is 22 of 30, or 73.3%. Three use July 1 without authority, two convert a per diem internally, one overlaps staff time, one lacks an adopted rate source, and one uses CBHT for a different service.

Keep Home Support Services Group separate

SPA 26-0001 also adds a Home Support Services Group description. Do not merge that coverage change into the CBHT unit conversion. Maintain separate rows for service definition, population, provider, setting, treatment plan, unit, rate, authorization, and claim. If one member receives both services, verify nonduplication and document each service's distinct purpose, time, staff, and outcome.
