An ABA stimulus fading worksheet should show which cue dimension was changed, the fading step actually presented, and whether the target response occurred under an enhanced, intermediate, or natural cue. A single accuracy total can hide control by the temporary prompt.
Use this ABA stimulus fading worksheet to define a case-specific stimulus-prompt sequence, retain invalid opportunities and access conditions, and keep natural-cue performance separate from prompted-step responding. The fictional example demonstrates the arithmetic without selecting a stimulus manipulation or clinical criterion.

Clinical and access boundary: Stimulus fading belongs inside an authorized, individualized process led by qualified professionals. Preserve readable, perceivable, motorically accessible, and meaningful speech, AAC, visual, auditory, tactile, environmental, and other supports. Do not make communication, safety information, mobility cues, or basic access harder to use merely to remove a prompt. Respond to help, break, stop, refusal, pain, fatigue, sensory distress, and alternative communication. This worksheet cannot establish consent, assent, medical necessity, effectiveness, mastery, payer coverage, accessibility compliance, or legal compliance. Stimulus fading changes a defined feature, not “help” in general
Stimulus fading changes a feature across steps so responding can come under control of the intended natural cue. The feature might involve size, contrast, position, shape, marks, intensity, or another dimension. Record values precisely enough to reproduce the display.
This differs from modeling or physically guiding the response, changing the task, removing access supports, or waiting longer before prompting. When packages combine procedures, name each component.
Stimulus Control Research and Practice discusses stimulus disparity, salience, and fading in discrimination training. It supports attention to controlling dimensions without selecting a cue, establishing accessibility, or setting a learner’s pace.
Ask which display, cue, and access conditions were present when the response occurred.
Meaningful purpose and discrimination card
Define the natural outcome and the discrimination before designing a prompt. A visually distinctive temporary cue can produce correct selection without the intended relation. If the final environment will not contain the same materials, people, positions, or display, state what must transfer.

Do not infer salience from what seems obvious to a clinician. It can vary with the person, device, lighting, distance, fatigue, and learning history. For communication symbols, involve the user and relevant AAC professionals before altering a display.
The natural cue must remain accessible at the end. Fading toward an inaccessible display is not successful transfer.
Define the cue dimension and ordered displays
Record one row per planned fading step. Include the exact change from the preceding display and the rule for advancing, holding, or stepping back. Preserve examples or approved screenshots only when privacy, copyright, and record-retention requirements allow.

An extra-stimulus prompt adds a feature outside the final cue, such as an arrow. A within-stimulus prompt exaggerates a target feature, such as size. Neither label replaces a concrete display description.
The study Thematic Matching as Remedial Teaching distinguishes stimulus from contingency manipulations and describes transfer in a specific symbolic-matching context. Its small sample cannot establish a universal sequence. Write the dimensions rather than relying on the method name.
Protect communication and sensory access before collecting data
Stimulus fading can unintentionally make information less visible, less discriminable, harder to reach, or more cognitively demanding. Treat accessibility as part of validity, not as a courtesy added after the trial.

The ASHA AAC Practice Portal describes AAC as individualized and multimodal. A report involving stimulus-size fading on speech-generating-device symbols illustrates one case-specific arrangement, not a reason to shrink, move, or reduce contrast on another person’s display without access assessment and user involvement.
An access support is not automatically an irrelevant prompt. Record whether it is expected to remain in the natural environment.
Opportunity ledger: keep the fading step attached
One row represents one planned opportunity. Capture the display, prompt value, natural cue, initial response, latency, access, and correction. A wrong or unverifiable display is invalid under the written rule, not a learner error.

Do not recode a corrected response as initial. Preserve added response prompts separately; a correct response after pointing, modeling, or guidance answers a different question from the uncorrected initial response.
Retain invalidity. A provider-side display error should not become evidence about the learner.

The natural-cue row deserves its own denominator. Combining eight correct enhanced-cue trials with six correct natural-cue trials creates 14 correct responses, but it does not show that 14 responses occurred under the natural cue. Report both the full-step descriptive view and the natural-cue view.
Retain target sets, display positions, partners, or contexts when they matter. Averages can conceal position control, an unusually easy target, or a display that only one implementer presents accurately.
Advance, hold, and step-back decision log
Record the prospective rule, evidence, person’s experience, and authorized decision. A step back may reflect an overlarge stimulus change, unstable access, an unclear natural cue, or implementation mismatch rather than learner failure.

Do not change the prompt dimension and the mastery rule at the same time without recording both changes. Do not move forward solely because a calendar date arrived. Do not hold a person at an enhanced cue indefinitely merely because prompted accuracy looks high.
Bound the decision to the target, display, cue dimension, version, people, and context reviewed.

Carry raw counts next to percentages. One of one is not equivalent to 20 of 20. Record sequential exposure because later steps occur after earlier teaching; cross-step percentages are not randomized causal comparisons.
Fictional makerspace label-sorting example
Noah, a fictional participant, helped select a community makerspace routine for sorting reusable tool labels. The team defined a relevant symbol feature, an accessible response, three display steps, help and stop communication, and a prospectively written advance/hold/step-back rule. This example tests recording and arithmetic, not the clinical choice.
The team planned 30 opportunities. Three were invalid: one used the wrong intermediate display, one occurred with glare that obscured the natural cue, and one began after the symbol board was moved outside the defined reach area. There were 30 − 3 = 27 valid opportunities, for 27 / 30 × 100 = 90.0% valid coverage.
Eight valid opportunities used the enhanced display, with seven initial correct responses. Ten used the intermediate display, with eight correct. Nine used the natural cue without the temporary enhancement, with six correct.

The 21 correct responses across all steps produce a planned correct yield of 21 / 30 × 100 = 70.0%. The three invalid opportunities are 3 / 30 × 100 = 10.0% of planned delivery. That 10.0% is not an error rate.
The overall 77.8% is descriptive, not evidence that the natural cue controlled 77.8% of responding. The natural-cue result is six of nine valid opportunities, with its small sample and context. Sequential, potentially unequal opportunities cannot prove that fading helped or harmed performance. Read Noah’s feedback, access record, target-level data, and follow-up alongside the counts.

In the fictional sample, 15 observations across nine components created 15 × 9 = 135 component opportunities. Reviewers scored 126 correct components and nine mismatches, which reconcile to 135. Descriptive fidelity was 126 / 135 × 100 = 93.3%.
Inspect the mismatches by component. A wrong prompt magnitude can change the fading step. A missing natural cue can make the response uninterpretable. An inaccessible board can invalidate the opportunity. Failure to honor stop communication is not repaired by eight other correct components.
Research comparing conditional discrimination after stimulus shaping, stimulus fading, and trial-and-error training reported procedure-dependent acquisition patterns in a historical study. Earlier research on visual position discrimination and stimulus fading also used narrow tasks and older terminology. These studies provide foundational evidence that procedure details matter; they do not create a modern default or expected outcome.
Fidelity asks whether the planned procedure occurred; appropriateness remains a separate decision.
Keep specialized applications within their own scope
Stimulus fading appears in varied packages, which can tempt broad claims. A study of differential reinforcement with stimulus fading during medical-exam routines involved four participants, function-based assessment, and specific exam steps. Those findings cannot support generalized compliance, removal of medical consent, or persistence through distress.
Likewise, a stimulus-fading method used with AAC, food, medical procedures, or safety cues may require speech-language, occupational-therapy, medical, feeding, swallowing, sensory, accessibility, or legal expertise beyond the behavior analyst’s competence. Keep those decisions with the qualified people who hold them.
The Ethics Code for Behavior Analysts addresses client rights, best interests, competence, informed consent, assent when applicable, confidentiality, documentation, and data use. Reopen the plan when the natural cue is not gaining control, errors cluster after one change, display fidelity drifts, access differs across steps, correction prompts become frequent, the person asks for a change, or follow-up does not support the meaningful outcome. The team may alter the cue dimension, make steps smaller, return to an earlier display, redesign the natural cue, select another teaching arrangement, seek interdisciplinary input, pause, or stop.
Preserve prior versions and supporting evidence. A new display is a new condition, not a correction to old data.

The sources differ in participant, target, stimulus dimension, prompting package, context, design, and outcome. They support explicit cue descriptions, step-level measurement, access checks, natural-cue tests, fidelity, and bounded interpretation.
