An ABA least-to-most prompting worksheet should show what the natural cue was, how long the team waited, which support first produced the defined response, and whether the opportunity was actually interpretable. A completed task alone cannot show independence: completion after a gesture, model, verbal cue, visual aid, or physical prompt is prompted completion.

Use this copyable ABA least to most prompting worksheet to review a case-specific arrangement. It is a review aid, not a universal prompt ladder. The fictional example keeps invalid opportunities, prompt use, independence, and fidelity in separate denominators.

Clinical, access, and contact boundary: A qualified team must determine whether this teaching arrangement is meaningful, assessed, authorized, within scope, accessible, and safe for the person and setting. Preserve ordinary communication, AAC, sensory, mobility, and other access. Explain the plan accessibly and respond to assent, dissent, refusal, freezing, pulling away, pain, fatigue, help, break, and stop communication. A worksheet cannot authorize physical contact, restraint, blocking, forced compliance, or removal of access. Current BACB ethics resources provide professional context but do not choose an individualized procedure.

Least-to-most is a case-specific sequence, not a universal hierarchy

In a least-to-most arrangement, the natural cue and planned wait come first. If the defined response does not occur, the implementer provides the first programmed support, waits according to the plan, and escalates only when the written condition is met. The first effective level is the least support in that particular sequence that produced the response on that opportunity.

The labels “verbal,” “gestural,” “model,” “visual,” and “physical” do not create a universal ranking. A spoken prompt may be less accessible or more intrusive for one person than a visual model; a visual cue may be ordinary access rather than a prompt to remove. Define the actual topography, purpose, and order instead of relying on labels.

The BACB test-content outlines and Sixth Edition BCBA Test Content Outline place prompting, prompt fading, stimulus control, measurement, maintenance, and generalization within behavior-analytic practice. Neither source prescribes one sequence or declares one prompt type least intrusive for everyone.

Meaningful skill and authorization card

Begin with the person’s goal and the natural outcome. Staff convenience, task completion, or a payer label does not establish that a target is meaningful.

Task performance cannot establish medical, motor, feeding, swallowing, communication, or accessibility safety. Seek the relevant qualified professional when those questions arise.

Define the natural cue, response, and valid opportunity

Write definitions another trained observer can apply. Avoid vague labels such as “cooperates,” “tries,” or “needs help.” Do not require eye contact, stillness, speech, or a particular body posture unless it is genuinely necessary to the meaningful outcome and justified.

The natural cue must be present and accessible before the initial response window begins. Missing AAC, the wrong materials, an implementer giving the answer early, an interruption, an undocumented prompt, or a safety stop may make an opportunity invalid under the prospective rule. Preserve the row and its reason; do not transform an implementation problem into a learner error.

Build the sequence from assessed, reproducible supports

Name the topography of each level and the transition rule. “More help” is not reproducible. If a level includes contact, document the authorized site, duration, force boundary, release rule, and immediate stop condition elsewhere in the plan.

The order belongs to this person, target, context, and version. Do not copy it to another person without assessment. A support that remains part of everyday life, such as a schedule, hearing device, adapted utensil, screen reader, or AAC system, may be an accommodation or natural cue rather than a prompt.

The ASHA AAC Practice Portal describes AAC as individualized and multimodal. Keep the person’s communication system available. Do not guide a device response and record it as independent communication, preference, assent, or consent.

Prewrite escalation, correction, and stop rules

Least-to-most does not mean automatically moving through every level. Define the conditions before observing the result.

Escalation should not compete with communication. If the person asks for help, a different method, a break, or an end, follow the individualized response plan. Never continue through a sequence merely to complete a data row.

Opportunity ledger: record first effective and highest support separately

One row represents one planned opportunity. Capture the natural-cue result before any programmed prompt, then the sequence actually delivered. The first effective level and highest delivered level can differ when a prompt is repeated, changed, or followed by correction.

Do not overwrite an initial error with the corrected outcome. If the response occurs after Level 2, code Level 2 as the first effective support even when the final task looks fluent. If the opportunity becomes invalid, retain all safety, communication, and implementation facts while excluding it from learner-outcome denominators defined for valid opportunities.

Report raw counts with percentages. Five independent responses mean something different out of six valid opportunities than out of 60. Sequential observations are not randomized comparisons, and a changing target set, context, implementer, or exposure history can change what the numbers mean.

Fictional accessible library self-checkout example

Maya, a fictional library volunteer, selected learning an accessible self-checkout routine. Her team defined the natural kiosk cue, accepted touch-screen and switch-access responses, ordinary visual and AAC supports, three case-specific prompt levels, wait and escalation rules, and help, break, stop, and invalidity definitions. This example tests arithmetic only; it does not recommend the target, prompt order, or decision threshold.

The team planned 36 opportunities. Four were invalid: one began with the switch interface disconnected, one used the wrong kiosk screen, one included an early undocumented prompt, and one was interrupted by a system reset. There were 36 − 4 = 32 valid opportunities, or 32 / 36 × 100 = 88.9% valid coverage.

Among the valid opportunities, 15 responses occurred independently after the natural cue and planned wait. Seven first succeeded at Level 1, six at Level 2, and four at Level 3. The categories reconcile: 15 + 7 + 6 + 4 = 32.

Seventeen valid opportunities used at least one programmed prompt: 7 + 6 + 4 = 17, or 17 / 32 × 100 = 53.1%. Planned independent yield was 15 / 36 × 100 = 41.7%, which keeps the four invalid opportunities visible. It would be inaccurate to call all 32 completed opportunities independent.

These sequential data are descriptive. They cannot show that the sequence caused learning, that a level was necessary, that a prompt was acceptable, or that the same order should continue. Review target-level patterns, latency, experience, access, and follow-up alongside the totals.

In the fictional review, 16 observations across nine components created 16 × 9 = 144 component opportunities. Reviewers scored 132 components correct and 12 mismatches; 132 + 12 = 144. Descriptive fidelity was 132 / 144 × 100 = 91.7%.

Inspect mismatches individually. An early prompt can erase the natural-cue test. A missing device can invalidate access. An out-of-plan contact or unhonored stop request is not diluted by a high overall percentage. Fidelity asks whether the written sequence occurred, not whether it was appropriate or effective.

Ask in an accessible form and allow delayed, nonvocal, device-based, or partner-supported communication. Absence of resistance does not establish assent. Do not make food, water, bathroom access, mobility, communication, relationships, health care, safety, rest, or emergency help contingent on completing a teaching sequence.

What research can and cannot tell this review

Research on receptive vocabulary instruction using least-to-most and most-to-least prompting and a comparison of prompting assessments suggests that outcomes may depend on learner, task, and arrangement. An earlier comparison of most-to-least and least-to-most prompting also used a narrow participant group and task. These sources support measuring the actual procedure and learner-specific outcomes, not claiming a universal winner.

A small study of parent preference for prompting procedures highlights acceptability as evidence worth collecting. Preference data do not substitute for the person’s own communication, individualized assessment, or legal consent. The Ethics Code for Behavior Analysts addresses rights, competence, consent, assent when applicable, documentation, confidentiality, and data use; those responsibilities remain with qualified practitioners.

Reopen the arrangement when independence is not increasing, prompts cluster at one target, escalation is inconsistent, latency or burden rises, ordinary access changes, the person requests a change, or maintenance and generalization do not support the meaningful outcome. Options may include changing the task, clarifying the cue, adjusting materials, reassessing prompt effectiveness, choosing another teaching strategy, seeking interdisciplinary review, pausing, or stopping.

Preserve the earlier version and its data. A revised sequence is a new condition, not a retroactive correction to old observations.
