An ABA frequency and duration data sheet should preserve more than a tally. A count becomes interpretable only when the reader can see the actual observation time, and a duration total needs clear event boundaries. The copyable form below keeps raw events, elapsed observation, missed coverage and calculated summaries connected.

Use this as a working draft under a qualified clinician's measurement plan. The worksheet does not select a target or measurement system, establish observer accuracy, identify behavioral function, prescribe treatment, or decide whether a change is clinically meaningful. This ABA frequency and duration data sheet keeps the components needed to review each calculation.
What the sheet records
The sheet has three connected parts. The event log preserves responses under a written start and stop rule. The coverage log shows when observation was planned, when it actually occurred and why any portion was invalid. Only then does the summary report count, rate, total duration, mean duration and percentage of valid observation time.
These measures answer different questions. Count is the number of recorded events. Rate relates count to time observed. Total duration adds the elapsed time of defined events. Mean duration divides that total by the number of complete events. Duration percentage relates event time to valid observation time. None of these figures explains why the behavior occurred.
Choose the response dimension before collecting data
Start with the clinical question, then ask which response dimension would answer it. A discrete response with observable beginnings and endings may support event count and rate. Duration may matter when episodes vary in length. Some targets need more than one dimension; extra columns still cannot rescue an unsuitable system.
The current RBT Test Content Outline, 3rd edition lists continuous procedures such as frequency, duration, latency and interresponse time, plus summaries such as rate and mean duration, among entry-level technician examination content beginning in 2026. It also addresses objective documentation, escalation of data irregularities, competence and supervision. An examination outline does not authorize independent method selection or supply a universal form.

Blank event-level record
Enter one row for each event that meets the definition. Preserve exact timestamps when the approved workflow allows them. If a start or stop is not observed, mark the duration unavailable instead of estimating it after the session.

Use a consistent time unit. If the timer exports milliseconds, preserve the raw value and document the conversion used in the report. When two defined responses can occur at once, the responsible clinician should state whether their durations may overlap. Do not silently force overlapping events into a single sequence.
Record actual observation coverage
The appointment clock and the observation clock often disagree. A 60-minute visit may include arrival tasks, privacy interruptions, unavailable opportunities, timer failures or a required pause. Rate and duration percentage use only the time during which the chosen observation rule was actually operating.

The article On Terms: Frequency and Rate in Applied Behavior Analysis discusses ambiguity in the word frequency and argues for retaining the time base with a response count. This worksheet labels count and rate separately. If a local plan uses frequency to mean rate, document that vocabulary in the measurement contract so another reader does not mistake a raw count for responses per unit time.
Calculation panel
Calculation comes after the validity review. Exclude time only when the written rule identifies it as invalid, and preserve the reason. An unusual or difficult result remains in the data when the observation itself was valid.

Count may include an event whose duration is unavailable if the start met the counting rule. Mean duration uses only events with complete, valid durations and must show that smaller denominator. If different opportunities or observation conditions make sessions incomparable, retain those differences rather than treating rate normalization as a cure.
Review data integrity before interpretation
Toward an Understanding of Data Collection Integrity discusses the importance of selecting a system that captures a relevant response dimension and the risks present in human-recorded data. A form can support an implementation check, yet the form cannot prove that an observer saw every event or applied a definition consistently.

Research comparing algorithms for observer accuracy in continuous recording shows that results can vary with the comparison procedure and characteristics of the recorded behavior. A broad agreement percentage cannot certify the event record as accurate. If interobserver agreement is collected, keep observer assignments, independence, comparison windows, formula and exclusions with that separate record.
Fictional completed example
Case M and every event below are fictional. For this arithmetic example, the target is “an open hand contacting the top of the desk with an audible sound, counted as a new event after five consecutive seconds without that contact.” The definition has no clinical meaning outside the example.
Four 15-minute observation windows were planned. Windows 1, 2 and 4 were completed under the stated rule. Window 3 was missed because the assigned observer was unavailable. The missing window was not entered as 15 minutes with zero events.

Coverage is 3 completed windows divided by 4 planned windows, or 75.0%. Valid observation time is 45 minutes, equal to 2,700 seconds. Rate is 6 divided by 45, or 0.133 events per minute when rounded to three decimals.
The mean duration is 290 seconds divided by 6 complete events, or 48.3 seconds. Duration percentage is 290 divided by 2,700, multiplied by 100, or 10.7%. Those two duration summaries use different denominators and should retain their labels.
The example supports a descriptive sentence: six defined events were recorded during 45 valid observation minutes, with 290 total seconds of event duration. Those totals do not show whether the observation windows represented the person's routines, whether the data were accurate, why the events occurred or whether treatment changed behavior.
Keep rate per time separate from rate per opportunity
This worksheet calculates events per minute. A plan may instead ask how often a response occurred when a defined opportunity was available. Those denominators are not interchangeable. Ten requests across 20 opportunities and ten requests across 60 minutes can both be useful, yet they answer different questions.
The review of continuous recording and interobserver agreement reporting describes standard time-based measures used in observational research and variation in agreement methods. That review does not make one denominator appropriate for every applied question. Use the denominator named in the current plan and retain the raw components needed for review.
Corrections and version control
Preserve the original entry. Record why a correction was made, who made it and whether the summary changed. If the response definition, timer rule, opportunity boundary or observation schedule changes, start a new version boundary. Recalculating older data under a later definition requires an explicit, reviewable decision.

The BACB Ethics Codes hub identifies the controlling materials. The linked Ethics Code for Behavior Analysts, updated August 2024 when checked, addresses competence, confidentiality, documentation, assessment, consent, correct implementation of data collection and continual evaluation. This worksheet cannot determine whether a particular record, correction or service meets those obligations.
Privacy, security and record boundaries
Use the record system, access roles and retention process approved for the actual organization and information. The HHS Security Rule risk-analysis guidance describes risk analysis for regulated entities and explains that the rule does not prescribe one universal method. HHS does not approve this sheet, a spreadsheet, a personal device or a transmission method.
The event log is not automatically a clinical note, incident report, treatment plan, authorization record, claim or official graph. Confirm which record is required, where raw data belong, how corrections are retained and who may access or disclose them.
Qualified review and practical fit
The CASP ABA Practice Guidelines access page identified Version 3.0, released in 2024, when checked. This article links to the access page without reproducing licensed guidance. Consult the current licensed material, plan, organizational policy, law, payer requirements and professional standards directly when they apply.
A measurement plan must fit the client, setting, clinical purpose and observer resources. The declared evidence supports cautious implementation questions and reviewable training evidence rather than a universal training package.
Stop and route concerns
Pause data collection when the target, timing rule, observation period, observer role, client authorization, safety plan or record destination is unclear. Mark a timer failure, obstructed view or interrupted window openly. Never fill a missing period from memory simply to complete the sheet.
Use the responsible clinical, medical, safeguarding or emergency route for an immediate safety concern, acute health change, escalating distress, inaccessible communication or withdrawal of assent when assent applies. Observation does not outrank care. Do not remove ordinary supports, provoke a response or continue an unsafe condition to obtain a complete event.
